Ordered by stage of CKD journey: diagnostic and risk awareness, then disease-modifying education, then day-to-day management, then specific conditions and advanced planning.

A note from the clinician who built this

Patients who understand their kidney disease do better.

Not just emotionally. Measurably. Over two decades of caring for kidney patients, the single most consistent thing I have seen is this: when patients understand what is happening inside their bodies, they take their medications more reliably, they catch sick days earlier, they avoid the things that quietly hurt their kidneys, and they make better choices about dialysis and transplant when the time comes. They ask sharper questions. They bring their families along. They stay healthier, longer.

Most kidney patients are still handed a photocopied pamphlet from years ago, or pointed to a generic website. They deserve more than that, and so do you. Every interactive module in this library was built right here at Northern Nephrology & Hypertension to help patients understand their kidneys and take care of them. Each one was shaped by thousands of real conversations I have had with CKD patients in clinic. They are not generic content. They are designed to feel like sitting across from me, going over a patient's situation together: the patient enters their information, and we review it.

NNH Interactive publishes these modules free, under a Creative Commons license, to any nephrology practice or CKD-serving organization that wants to use them. Embed them in your patient portal, your practice website, or your education workflow. Apply your practice's branding via three URL parameters. Use them in exam rooms, hand them to patients to explore at home, or both. Activation costs nothing and works on every patient. That is a promise I plan to keep.
